Life Insurance

Basic Life & Accidental Death Insurance

We provide this plan at no cost to you. Your coverage amount is 1.5x your annual benefit earnings.

Supplemental Life Insurance

In addition to Basic Life Insurance, you may buy Supplemental Life Insurance for yourself, your spouse and children. See your Benefits Summary for details.

Evidence of Insurability

If you elect coverage within 30 days from your hire date or the date you become a full-time employee, you won’t have to submit Evidence of Insurability (EOI).

EOI is an application process that provides information on your health and your dependent’s health in order to gain coverage.

You may choose these amounts without submitting EOI:

  • For yourself: 5x your salary or $500,000, whichever is less
  • For your spouse: up to $50,000
  • For your children: up to $10,000 each

If you elect Supplemental Life Insurance for yourself or spouse after the initial eligibility period, EOI may be required before coverage can begin.

Disability benefits

Short-Term Disability (STD)
Worthington Industries provides disability benefits if you become disabled due to illness or accident. STD provides income protection for the first 26 weeks of disability. This plan is administered by New York Life, and we provide STD at no cost to you.

In addition, your MyQHealth Care Coordinators are here to assist you with filing your disability claim and answer any questions you may have. If you’re going on disability or FMLA leave, your Care Coordinators will guide you through the process every step of the way — just call 888.971.7377. Starting January 1, 2022, if you have questions about STD, LTD or leaves, contact Broadspire at 888.959.6594.

NOTE: Beginning January 1, 2022, Broadspire will be our new STD and leave administrator.

Long-Term Disability (LTD)

If your disability lasts more than 26 weeks, you may qualify for LTD benefits. It pays 60 percent of your highest earnings during the two years before LTD. Your benefits are offset by workers’ compensation and Social Security benefits up to age 65. LTD is administered by MetLife. Broadspire will automatically transition claims to MetLife for review.
